What is OFR?
The Lancaster County OFR is a multi-disciplinary process that reviews overdose deaths to understand the circumstances leading up to each fatality. The goal is to identify system gaps and missed opportunities for intervention and use those insights to strengthen community response and prevention efforts.
Goals of the OFR:
- Identify barriers to care and service gaps
- Analyze patterns and risk factors
- Develop actionable recommendations to improve systems
- Support local prevention strategies
Who Participates?
- Public health organizations
- Behavioral health and substance use providers
- EMS and healthcare systems
- Coroner’s office
- Social services and community-based organizations
- Law enforcement and corrections
How It Works:
OFR team members meet regularly to review de-identified overdose cases. Through shared information, the team identifies where systems can be strengthened and creates recommendations to improve service coordination, access to care, and prevention efforts.
